Global Perspectives is conducting a survey, sponsored by a pharmaceutical company, to better understand the impact of caring for someone with agitation in Alzheimer’s dementia.

We would like to hear from you if you care for someone with agitation in Alzheimer’s dementia and they are taking Rexulti to treat their agitation. Your participation would involve completing a 30-minute online survey.

We’re looking for people who help to care for someone with agitation in Alzheimer’s dementia (such as family members, friends, or other caregivers where it is not their employed job) to share their perspectives.

If you qualify and complete the survey, you will receive:

• $100 for a thank you for your time completing the 30-minute online survey

• Your voice is critical. By taking part, you will help researchers and health leaders better understand how caring for someone with agitation in Alzheimer’s dementia impacts on people’s everyday life and emotional well-being.
• By getting involved, you can make your voice heard and may influence key decision makers in the development of future treatment options.
• The survey findings may be presented at scientific conferences and published in a scientific journal.

• Your responses will remain anonymous. Your responses will never be presented on their own, they will always be grouped with responses from other people.
• Individual feedback about your answers will not be shared with Alzheimer’s Los Angeles.
• The only people who will see your individual responses are the small team analysing the data, they will not have access to any of your identifiable information (such as your name, email address or location).
• We will take great care to ensure that your information is kept safe and secure but there is always a very small risk of your data being accessed by an unauthorised person.
