Welcome to Enhanced Care Management (ECM) at Alzheimer’s Los Angeles

A Free Medi-Cal Program for Families Facing Memory Loss

Supporting patients and families with memory loss can be complex, but Alzheimer’s Los Angeles is here to assist you. Enhanced Care Management (ECM) is a free Medi-Cal program providing families with care management, personalized support, and resources. ECM helps your patients and families living with memory loss to remain safe, comfortable, and well-supported at home.

Referral Benefits. How ECM Can Assist Your Patients and Families:

  • Addresses memory loss in conjunction with co-existing health conditions such as diabetes or cardiovascular disease

  • Provides care management, including developing a care plan and connecting patients to primary and specialty care, scheduling appointments, and arranging transportation

  • Coordinates follow-up after hospitalizations or emergency department visits to support a safe transition back to the home environment

  • Delivers individualized services and support tailored to each family’s needs and circumstances
  • Conducts medication reviews to promote safety, adherence, and effectiveness

  • Offers assistance with applications for in-home care, nutrition benefits, and respite services to support caregivers

  • Facilitates educational programs and skill-building workshops for those supporting individuals living with memory loss or dementia

This program may be appropriate for patients who:

Are experiencing memory loss or dementia (a formal diagnosis is not required)

Need assistance with activities of daily living such as bathing, dressing, toileting, eating, or medication management

Are enrolled in Medi-Cal through Health Net or L.A. Care and reside at home in Los Angeles County

Have a caregiver or support person who can participate in the program on an ongoing basis

Dementia Care Manager (Bilingual English/Spanish)

The primary point of contact who collaborates with clients, caregivers, and an interdisciplinary team to develop person and family centered care plans. The Dementia Care Manager coordinates services and connects families to resources through Alzheimer’s Los Angeles and other community agencies.

Nurse Consultant

Provides clinical support to the care team by advising on issues such as falls, sensory impairment, skin integrity, and incontinence. The Nurse Consultant offers guidance on managing complex medical conditions and assists with advanced care planning.

Licensed Clinical Social Worker (LCSW)

Collaborates with the care team to address psychosocial needs, support the individualized care plan, and close gaps in care. The LCSW provides caregiver support, education on behavioral management strategies, and guidance to help prevent caregiver burnout, abuse, or neglect.

Geriatric Pharmacist

Specializes in medication management for older adults. The Geriatric Pharmacist conducts comprehensive medication reviews to ensure safety, appropriateness, effectiveness, and provides recommendations to optimize medications. The Geriatric Pharmacist also educates caregivers and encourages communication with prescribing clinicians regarding any medication concerns.
