Welcome to Enhanced Care Management (ECM) at Alzheimer’s Los Angeles

A Free Medi-Cal Program for Families Facing Memory Loss

Taking care of someone with memory problems can be hard, but you don’t have to do it alone. With Enhanced Care Management (ECM), a free Medi-Cal program, you’ll get the personalized support you need. ECM helps people living with memory loss stay safe and comfortable at home by working with family/friend caregivers to provide care navigation and support.

What ECM does for you:

  • Provides services and support that fit each family’s needs

  • Helps manage memory loss along with other health problems like diabetes or heart issues

  • Provides assistance finding doctors, making appointments, and arranging rides

  • Follows-up after hospital or emergency room visits

  • Reviews the person’s medicines to make sure they’re safe and working well

  • Provides help applying for in-home care, food benefits, or respite services (break for caregivers)

  • Sets-up classes for learning new skills to help you care for someone living with memory problems or dementia

This program may be a fit if the person you care for:

Has memory problems or dementia (they don’t need a diagnosis)

Needs help with daily activities like bathing, dressing, or remembering to take medicine

They have Medi-Cal with HealthNet or LA Care and live at home in Los Angeles County

You or another caregiver can be involved in the program on an on-going basis

Meet the team that’s here to help:

Dementia Care Manager

Your main contact who will work with you and a team of experts to create care plans that work best for your needs. Your Dementia Care Manager will help coordinate care and connect you with resources at Alzheimer’s Los Angeles and other community resources.

Nurse Consultant

Supports the team by sharing information about challenges like falls, hearing problems, skin sores, and bathroom accidents. The nurse consultant also helps with complicated health problems and making plans for future care.

Licensed Clinical Social Worker (LCSW)

Works with the team to provide support, treatment plans, and fill in care gaps. The LCSW teaches how to handle behaviors, caring for caregivers, and preventing abuse or neglect.

Pharmacist

Has a specialty focus on older adults. The pharmacist reviews medications to make sure they are safe, the right choice, and check that they work well together. The pharmacist teaches and encourages families to talk with their doctor about any medication concerns.
