Medi-Cal Changes

Changes in Medi-Cal eligibility are in progress and coming soon. If you or someone you care for is enrolled in Medi-Cal, these changes may impact you.

Work Requirements

Starting January 1, 2027, many adults ages 19 to 64 will need to show 80 hours a month of work, school, or volunteering to keep their coverage. They will also have to renew every six months instead of once a year.

Some people are exempt, including kids, pregnant women, people 65 and older, and many family caregivers. If you care for a family member with dementia, you are usually exempt. In 2027, you can just say so, with no extra paperwork. But starting in 2028, you will likely need written proof, so it’s smart to gather things like a doctor’s letter now.

Immigration Status

Other changes affect adults with “unsatisfactory immigration status.” Since January 1, 2026, these adults can no longer enroll in full-scope Medi-Cal. If you already have coverage, you can keep it by renewing it on time.

Dental care ended on July 1, 2026, for most of these adults, except emergency care.

In July 2027, adults age 19 to 59 who stay on full-scope Medi-Cal will pay $30 a month. Children, pregnant women, and former foster youth are not affected.

What can I do now?

  2. Open and respond to Medi-Cal mail right away. You’ll receive notice of any changes or reporting requirements via mail. Missing deadlines can cause a loss of coverage.
